Viridian Therapeutics, Inc. (“Viridian”) offers mobile text messaging programs as part of the ViridianCares patient services program. These optional programs provide enrolled participants with support services related to Viridian products, including, but not limited to: online resources, financial assistance information, access and reimbursement updates, treatment support services, prescription reminders, and other materials or reminders related to such services.

By opting in to any ViridianCares text messaging program, you acknowledge and agree to these Terms & Conditions.

Enrollment in ViridianCares Text Messaging Programs

You may opt in to receive text messages from ViridianCares by following the enrollment instructions provided within the patient enrollment form or via your Patient Access Liaison through a Viridian‑approved enrollment channel.

By signing up to receive texts, you consent to receiving ongoing, recurring text messages from Viridian at the mobile number you provide. These messages may be sent using an automatic telephone dialing system or other automated technology.

After you enroll, Viridian will send you a confirmation text message to verify your participation.

Message frequency will vary depending on your program enrollment and interaction with ViridianCares but it is generally expected to be approximately 10-15 messages per month.

You represent that you are the owner or authorized user of the mobile number you provide.

Message and Data Rates; Service Availability

Viridian does not charge a fee for participation in the ViridianCares text messaging program. However, message and data rates may apply, depending on your mobile carrier and your subscription plan.

Program availability is subject to your carrier’s coverage and system availability. Viridian makes no guarantee that text messaging services will be available at all times or in all locations. Service may be affected by factors beyond Viridian’s control, including carrier issues or device compatibility.

Viridian reserves the right to modify, suspend, or terminate any text messaging program at any time without notice.

Unsubscribing from ViridianCares Text Messages

You may stop receiving ViridianCares text messages at any time by replying STOP to any message you receive. After your request is processed, you will receive one final text message confirming that you have been unsubscribed.

Privacy

Information collected through ViridianCares text messaging programs will be used in accordance with the Viridian Privacy Notice available at: viridiantherapeutics.com/privacy
